To take this survey you do not need a password. If this is your first visit to the survey site, do not type anything into the password box. Simply click on Begin Survey.If you are unable to complete the survey in one session, you will be able to click on a Finish Later button at the bottom of the screen. You will then be provided with a randomly generated password that will allow you to access your unfinished survey. Once you have submitted your survey, your password will be deactivated
